Our associated providers and pharmacy partners operate under Business Associate Agreements governing how PHI may be used and disclosed. Infinity Health MD is a platform that connects patients with independently licensed healthcare providers offering evaluation and, where clinically appropriate, treatment across Weight Loss, Hair, Skin, and Peptides. Infinity Health MD does not itself practice medicine; clinical decisions are made solely by licensed providers.
Infinity Health MD is not an emergency service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Plans are billed on a recurring monthly basis unless otherwise stated. You may cancel at any time; cancellation takes effect at the end of your current billing period. Prices are subject to change with notice.
Content on this site is for informational purposes and is not a substitute for professional medical judgment. Only a licensed provider reviewing your specific case can determine what treatment, if any, is appropriate for you.

This Medical Consent describes what it means to receive care through Infinity Health MD's telehealth platform. By completing your assessment and proceeding with treatment, you acknowledge and agree to the following.
Telehealth involves the use of electronic communications to enable a licensed provider to evaluate, diagnose, and treat you without an in-person visit. Your provider will review your assessment, medical history, and any photos or lab work you provide to determine whether treatment is appropriate.
Telehealth offers convenient access to licensed providers without the need for an in-office visit. As with any care, there are limitations: your provider cannot perform a physical exam in the traditional sense, and some conditions may require in-person evaluation, additional testing, or referral to a specialist. Your provider will let you know if telehealth isn't appropriate for your situation.
If your provider determines that prescription treatment is appropriate — including medications related to Weight Loss, Hair, Skin, or Peptides programs — they will explain the treatment, its intended use, common side effects, and any monitoring (such as lab work) required. You are free to ask questions and decline treatment at any time.
Information shared during your telehealth visit is confidential and protected in the same manner as information shared during an in-person visit, as described in our Notice of Privacy Practices.
You may withdraw your consent to telehealth treatment at any time by notifying your care team. Withdrawing consent will not affect your ability to seek care elsewhere.
Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not rely on telehealth messaging for urgent or emergency needs.
You will review and sign this consent with your care team before your first telehealth visit.
